Psychiatry Office
We have 1 M.D., 2 Nurse Practitioners, 1 Ph.D. and 5 therapists. We provider medication management and individual therapy. We treat a variety of conditions: mood disorders, anxiety disorders, PTSD, other mental disorders such as, : schizophrenia, Bipolar, ADHD, and substance abuse.

Welcome New Patients
Please bring your completed New Patient Packet with you to your first appointment completely filled out. If you are a returning patient, please complete the Update Form to tell us what, if anything, has changed.

Your First Visit:
To care for you as efficiently as possible, please bring .the following items with you to your first new patient visit:
- Photo ID
- Insurance Card
- Pharmacy Card
- Co-Payment
- Complete list of all current medications, including over the counter. Include mg and how you take medication.
- Completed New Patient Packet
- Prior medical records, if available.
New patients should arrive 15 minutes early to your scheduled appointment to complete and necessary information for our system if you have completed the packet. If you have not completed the packet please arrive 30 minutes early to give you time to complete the packet. Bring completed packet to front desk check in when you arrive.
For subsequent visits, please bring the following:

Returning patient should arrive 15 minutes early to your scheduled appointment to complete the check in process. To expedite the registration process as a returning patient please tum in the completed form at check in upon arrival at-the office.
Patient After Office Hours Instructions
Medication Refills
It is important that you keep track of your medication refills and call our office during our regular business hours to refill your medications. Our policy is that you will receive a script for only enough medication to get you to your next scheduled appointment.
If you are unable to call us during our office hours, you will need to call the office and leave a message with the answering service and we will contact you on the next business day.
Appointments
Appointments will only be scheduled during our office hours. If you are calling after hours, please leave a message with our answering service and We will return your call on our next business day.
Illness & Medical Emergency
Our office does not prescribe any medications after office hours. If you are sick or feel that you are experiencing a medical emergency, please call 911 or go to your nearest emergency room or urgent care for treatment.
